Due to changes in Federal Regulations, the Federal Drug Testing Custody and Control Forms have been revised.

If you are currently using the ‘old’ Federal Custody and Control Forms, please know that you will be required to use the new ones now.

Here is a guide to what revisions have been made on the Federal Drug Testing Custody and Control Forms. Please note that these revisions apply to DOT and Federally-regulated testing only.

If you are a current Fleetworthy Solutions customer, that this applies to, please know there are a few options for shipment of the new 2018 revised Federal Custody and Control Forms to your location. Please contact us to coordinate shipping. **It is imperative that when the new Federal Custody and Control Forms are received that the old forms are disposed of.

** If an expired Federal Custody and Control form is used, you must provide a signed statement (i.e., a memorandum for the record). It must state that the incorrect form contains all the information needed for a valid DOT drug test, and that the incorrect form was used. The statement must also list the steps you have taken to prevent future use of an expired Federal form for DOT tests. You must supply this information on the same business day on which you are notified of the problem, transmitting it to the MRO immediately.
